Akut kolesistit safra kesesiin inflamatuar bir illetidir. En çok safra kesesi taşları nedeniyle oluşur. Şayet enflamasyondan sonra bakteri invazyonu meyda­na gelirse asenden kolanjit, ampiyem yahut amfizamatöz kolesistit üzere illetler meydana gelebilir.

Akut Akalkülöz Kolasistit Nedir?

Safra kese taşı olmadan gelişen safra kesesi iltihabıdır. Majör risk faktörleri ileri yaş, önemli hastalık durumu, yanık, travma, majör cer­rahi, uzun devir total parenteral beslenme, diyabet, immün süpresyon ve veladet yapmış olmaktır.

Safra kesesi perforasyonu akut kolesistitlilerin %10’ unda meydana gelir. Perforasyon nahiyesinde komşu kolesistik enterik fistüle, safra kesesi ileusuna ve safra içeriğinin karın boşluğunu etkilemesi sonucu perito­nite yol açabilir.
Kronik kolesistit acilen hep safra kesesi taşlarının yaptığı obtrüksiyona bağlı olarak tekrarlayıcı ataklar biçiminde gelişir. Tekrarlayan hasara bağ­lı olarak kese duvarında fibrotik kalınlaşmalar oluşur ve ahir bakteriyel enflamasyon ya da akut kolesistit olmaksızın gerçekleşen bu safra kese distansiyonu hidrops kese olarak isimlendirilir.

Porselen kese; kronik kolesistit enfeksiyonu sonrası fibrozisin yol açtığı geniş kalsifikasyonlara ise porselen kese olarak isimlendirilir.
Amfizamatöz kolesistit; umumide safra kese taşlarının sebep olduğu gaz üreten mikro organizmaların enfeksiyonudur. Amfiza­matöz kolesistitde süratli ve önemli klinik bozulma gerçekleşir, gangren ve/veya perforasyon gerçekleştiğinde mortalitesi %15’ lerdedir.

Safra kesesi taşlan klinik olarak sessiz taşlardır ve çoklukla diyagnostik yahut öteki bir hedef için yapılan görüntüleyici testler esnasında tesadü­fen fark edilir. Tespit edilen taşların ise komplikasyon ve ağrı riski her bir yıl için %l-%4 üzere epey düşük seviyededir. Yalnızca %10 hastada birinci 5 yıl, ve
%20 hastada birinci 20 yıl ağrı yahut taş komplikasyonları gelişecektir.

Safra kesesi çamuru ; çoğunlukla kolestrol monohidrat kristalleri, kalsi­yum bilirubin pigmenti ve başka kalsiyum tuzlarından meydana gelir. Safra kesesi çamuru formasyonunda ana mekanizma muhtemelen kese motilitesinde bozukluktur. Safra kesesi çamuru abdominal USG de saptanabilir. Kli­nik gidişi külliyen gerileyebilir yahut taş oluşumuna ya da sair komplikasyonlara kadar çeşitlilikte olabilir. Asemptomatik safra kesesi ça­muru ya da mikrolitiyazisi olan hastaların tedaviye ihtiyaçlan yoktur. Safra kesesi çamuruna bağlı semptom yahut komplikasyon gelişiminde ise hasta­larda kolesistektomi göz önünde bulundurulur. Safra kesesi çamuru oluşu­mundan korunmak için ise her hangi bir kanıtlanmış metot yoktur.

Safranın Özellikleri:
Safra bileşimi %80 su, %10 safra asidi, %4-5 leştin ve başka fosfolipidler, %1 kolesterol ve vesair geri kalan kısım ise konjuge bilirubin, elektrolit­ler, mukus ve çeşitli proteinlerdir. Çokça kolesterol direk olarak safraya salgılanır yahut safra tuzlarına çevrilebilir. Salgılandıktan sonra safra intrahepatik ve ekstrahepatik safra kanalları ile safra kesesine gelir. Burada safra kesesinin ana fonksiyonu safradaki su ve sodyumu absorbe ederek safrayı konsantre hale getirmektir. Uzun süren açlık ve safra kesesinde rezidüel volüm kalması safra kesesi taşlarının oluşma riskini arttırabilir. Safra kesesi kontraksiyonunu bozan durumlar ise hamilelik, obezite, süratli kilo kaybı, diyabet ve total parenteral nutrisyondur. Taş safra kanalına düştüğünde ise ağrı ve kusma intra lüminal basınçtaki artışa bağlı olarak meydana gelir.
Biliyer sistemdeki bir taşın meydana getirdiği obtriksiyon persistan kontraksiyonlar ile giderilemez ise ve taş ile kanal tıkanıklığı devam ederse mekanik, kimyasal ve infektif enflamatuar yanıt gerçekleşir ve iskemi mey­dana gelir. Tıkanıklık esnasında bakteri ve pankreatik enzim içeren intestinal içeriğin biliyer kanala reflüsü meydana gelebilir. Enfektif ajanlar olarak vakaların %74’ünde Gr (-) bakteriler (E.coli %36, klebsiella %15) tespit edilmekte, %15 orantısında ise Gr(+) bakteri­ler (Enterococcus, 6%; Staphylococcus, 3%; and Streptococcus, 2%) saptan­maktadır. Bacteroides (5%) and Clostridium (3%) ise daha nadir sapta­nan enfektif ajanlardır.

Akut Kolesistit Klinik Görünümü

Klasik sağ üst kadran ağrısı distandü safra kesesinin peritona teması ile olur ve ağrı karnın öbür ortamlarına ve/veya epigastriuma, bele sırta, skapulaya, sağ omuza yayılabilir. Ağrının sol sırta yayılması, safra kesesi bozukluklarıyla bağlıdır. Ağrı öğünlerle ya da yağlı azık intoleransı ile bağlı olmak zorunda değildir. Hastalarda bulantı, kusma, terleme, ateş üzere semptomlar bulunabilir. Bulantı, kusma ve ağrı çoklukla ortak safra kanalının distansiyonuna bağlı olarak meydana gelir. Dispepsi, geğirme, şişkinlik ve yağlı azıklara intolerans kolesistektomi geçirmiş safra kesesi olmayan hastalarda sıktır.

Biliyer kolik ağrısı ekseriyetle taşların safra kesesi ve/veya biliyer traktı tıkaması ile anı gelişen distansiyon sonucu meydana gelir. Ko­lik ağrı umumiyetle 1-5 saat arası sürer ve sonrasında azalır. Enfeksiyo­nun eşlik gelişmediği hallerde şayet taşlar spontan olarak safra kesesi lümenine geri sarfiyat ya da ampulladan duodenuma düşerse ağrı resen kaybolur. Biliyer kolik ağrısı 5 saat üzerinde devam ed,yorsa akut kolesistit, assendan kolanjit yahut pankreatit üzere kompli­ kasyonlardan şüphelenilmelidir.

Akut Kolesistit Tanısı;
Acil serviste kıymetli olan birinci evvel komplike olmayan biliyer kolik ağrısı ile akut kolesistit ve komplikasyonlarının ağrısını ayırt etmek olmalıdır. Akut kolesistitin tanısı anamnez, fizik muayene, laboratuar ve görüntüle­me sistemlerindeki bulguların kombinasyonlarına dayanarak bulunur.

Laboratuar bulguları akut kolesistiti gayrı durumlardan ayırt edemez.
Lökosit sayısı >10,000/mm3 olması tanı koymada %63 sensitivite ve %57 spesifıte ile LR 0.6 pahası vardır. Ortalama lökosit sayısı 12,600/mm3
kolesistit olan hastalarda bilinmeyen nedenden karın ağrısı olan hastaların %26’ sı ve gastroenteriti olan hastaların %43’ ünde olan sayılara benzeridir.
C-reactive protein seviyesinin 3 milligram/dL yükselmesi spesifik değildir fakat akut kolesistit tanısı ile bağlantılıdır.

USG bulguları ile CRP seviyelerinin kombine edilmesi sentiviteyi %97, spesifiteyi %76’ya çıka­rır. AST, ALT yahut alkali fosfataz yükseklikleri ortak safra kanalı taşı, asendan kolanjit yahut Mirizzi sendromunu (sistik kanaldaki taşın ortak hepatik kanala dışarıdan pres yapması) düşündürebilir fakat akut ko­lesistit için spesifik değildir.

Direk grafi safra kesesini göstermede çok zayıf bir gö­rüntüleme metodudur zira yalnızca %20 nispetinde safra taşları direk grafide görülebilinecek kadar kalsiyum içerir. Direk grafı amfizamatök kolesistit, kolanjit yahut kolesistik enterik fistüllere bağlı olarak oluşan kese duvarındaki yahut biliyer traktusdaki havayı saptayabilmesine rağ­men, bu saydığımız komplikasyonların klinik bulguları daha ön planda­dır ve öteki görüntüleme yolları daha kullanışlıdır.

USG akut kolesistiti sapta­mada sensivitesi %94, spesifitesi %78’dir. lmm’den küçük ve ekojenik olmayan taşları ile sistik ka­nal ve safra kese boynunda bulunan taşları saptaması çetin olabilir.Sonogrofık murphy ise sonogrofık probun safra kesesi üzerine bastırılma­sı ile maksimum hassasiyetin olmasına denir. Murphy işareti PPV %92 olup safra kesesi taşı varlığı ve öteki klinik bulgularla kombinasyonu PPV %87’ dir. Sonogrofık Murphy bulgusu diyabetik ve gangranöz kolesistitli hastarda olmayabilir. Gayrı klinik bulgular olmadan hiç­bir USG bulgusu tek başına akut kolesistit tanısında spesifik yahut sen­sitif değildir.Safra kese duvar kalınlığı 3mm’den büyükse kolesistit göz önünde bulundurulmalıdır. Lakin pankreatit, assit, sağ kalp yetmezliği ve alko­lik hepatit üzere durumlarda safra kesesi duvar kalınlığında artış meydana gelebilir. 3-5 mm arası kıymetler anormal bedellerdir lakin bir çok kole­sistit hastasında bu duvardaki kalınlaşma 5 mm’nin üstündedir. Periko­lesistik likit varlığı daha az saptanan bir bulgu olmasına karşın kolesistit için epey spesifiktir. Biliyer duktus çapı yaşla birlikte artmasına rağ­men ortalama 7mm civarındadır. Batın tomografisi görüntülemesi USG sonuçları şüphe­li ya da bilinmeyen olduğu durumlarda kullanılır.

Hepatobiliyer MRI; Magnetik resonans kolanjiopankreotografi’yi de içeren kapsayan MRI tetkiki ise CT ve USG’nin belgisiz olduğu ve akut kolesistitten şüphe­lenilen hastalarda uygulanabilir. Akut kolesistitde duvar kalınlığı artışının saptanmasında MRI en yüksek sensitivite nispetine sahiptir (sensitivitesi %100), perikolesistik likit varlığında ise %95 ve bitişik yağ sinyal intersitisindeki değişikliklerinin sensitivitesi ise %95’dir.

Safra kesesi taşı tedavisi

Komplikasyonlar açısından risk faktörleri içermeyen hastalarda (si­roz, portal hipertansiyon, Orak hücreli anemi ve transplant adayı olmayan) asemptomatik safra kesesi taşı varlığında tedaviye gereksinim duyul­maz. Umum cerrehi mütehassısına başvurmaları önerilmelidir. Biliyer kolik ağrısının tedavisinde parenteral opiat analjezikler ve ketorolak üzere nonsteroidler kullanılır. Cerrahi tedavi kesin tedavi için gereklidir. Semptomatik safra kesesi taşlarının kesin tedavisi için birinci tercih te­ davi laporoskopik kolesistektomidir.
Akut kolesistit ve komplikasyonlarının tedavisi için hastaneye ya­tış yapılmalı kolesistektomi açısından cerrahi konsültasyonu istenmeli­dir. Bu hastalarda laporoskopik kolesistektomi sık tercih edilen bir te­davi prosedürüdür. ERCP ortak safra kanalı yahut genişlemiş ortak ka­nal durumunda uygulanır.
